Conversion of Life or Basic Critical Illness Insurance 

If you are under age 65 at the time your benefit eligibility terminates, you have the option to convert your AFBS Group Life Insurance to an individual policy without providing medical evidence. A written request, which includes payment of premium, must be received within 30 days of the termination of your benefit eligibility.

Basic Critical Illness Insurance

Basic Critical Illness Insurance is underwritten by Industrial Alliance Pacific Insurance and Financial Services Inc. (IAP).  Members under age 65 who have been insured for Basic Critical Illness Insurance benefits continuously for at least 24 months, who have not received an AdvanceCare Benefit, and who are no longer eligible for the Insurance Program provided by AFBS, may apply to convert their coverage to an annual renewable individual Critical Illness Insurance policy. 

You must notify IAP directly of the conversion request within 31 days of termination of benefit program eligibility.  Coverage under any converted policy will terminate at age 75.

This provision does not apply if the policy which has been issued to AFBS by IAP is terminated.
