Submitting a Critical Illness Claim to Industrial Alliance

Basic Critical Illness benefits are provided under Industrial Alliance Insurance and Financial Services Inc.  Policy number 100006095 issued to AFBS.The adjudication and settlement of Basic Critical Illness claims is handled directly by Industrial Alliance.

The claim forms required by Industrial Alliance are dependent on the Covered Condition for which a claim is being submitted. Claim requirements can be obtained directly from Industrial Alliance by calling 1-800-266-5667 (option 1). In addition to information about the Covered Condition for which you are claiming. Industrial Alliance will require your AFBS Membership Number and policy number 100006095.
